An experienced management staff, including a Program Director and a Care Coordinator are available for referral intake. The Program Director or Care Coordinator visits and and informs the caregiver and patient about the program. There will be a detailed assessment and the team will develop a care plan with input from the caregiver, taking into consideration the specific needs of the patient. The care plan may include most activities that the caregiver would normally provide.
In addition, the Program Director or Care Coordinator addresses health and safety concerns. For example, the Falls Prevention Program is reviewed along with the home safety checklist.
